PLEASE READ CAREFULLY AND ENTIRELY

I understand the risk of personal injury while engaging in any form of martial arts training and exercise training and with this knowledge agree to assume the risk of any injury or damages I might incur during training. 

Specifically, I agree to hold harmless Vincent “Vinny” Panza, operating as Gorilla Vinny Training, from all losses, damages, injuries, causes of actions, claims, or complaints in the event that I suffer damages or injuries in any way during the participation, instruction, and/or performance of any training activity or during transit to or from training. Likewise, I agree to strictly obey any safety guidelines and/or instructions delivered to me regarding activities that occur during training or for when performing any martial arts or exercise related activities on my own.

Because of the physical demands of training, I understand that I must be in good physical condition to participate in training and I explicitly confirm that no medical professional has prohibited me from participating in exercise or martial arts training of any kind. I understand that in case of injury, Vincent “Vinny” Panza, operating as Gorilla Vinny Training, is not a medical doctor and is not expected to provide anything more than basic assistance.

I understand that during training, photographs, audio, or visual recordings may be taken with my consent and may be used for publication, promotion, articles, graphics, and advertisement without additional consent and without compensation at this time or any other time.

I have read and understand this release and agreement and agree to its provisions now and in the future. I am not under the influence of any drugs, alcohol, or other intoxicants and I am not suffering from any illness or incapacity.

STANDARD
LIABILITY WAIVER

To keep us all on the same page, please read and fill out the liability waiver below. It’s standard language for New York-based fitness businesses and is similar to any waiver you’d sign when signing stepping into a gym to do things like lift weights, take a yoga class, or hit a heavy bag. Please read carefully and sign at the bottom before our first session. Thanks!
